To download the forms to your computer, hold the option key and click on the link below (or right-click on a PC) labeled "Student Teacher Application Personnel Data Form". A window will appear, select "Save this Link as...", or some similar "Save as..." feature, from the menu. Then save the file to your local drive or to a disk. You will have to open the downloaded document from within Microsoft Word '97(PC) or '98(Mac). If you have problems contact the Education Department office.
